Actually I made even 2 visits to Polish embassy, becaues mine and my wife's surnames do not match, but still with a proof of our marriage certificate I was allowed to submit all needed papers for the visa solemnly by myself. BTW, now after we visited Poland with one-entry visa, next time we can apply for multi-entry visa, believe me. ps.
